The first document is the Battle Casualty Report issued 25 June 1945.
There is a notation that a telegram was sent to Mrs Forystek 28 June 1945.
Cpl Forystek's status is Missing in Action.

The second document is the Report of Death dated 7 December 1945.

This report is based upon a report from
the Japanese Government dated 20 November 1945.

This is intriguing because Cpl Forystek's remains
were not disintered from the Koishikawa Military Cemetery until March 1946.

At that time his initial identification was made in
the field based upon a laundry mark ( 9200),
the last four digits of his Serial Number.

This was an accepted process in the early days of recovery.

In addition, a year later a reinvestigation was launched
with a more detailed examination of his remains.

This most likely took place at the
Central Identification Laboratory in Yokohama.
