located with the blue circle at the top left corner.

At the bottom right, the blue oval shows the location of
the Eastern District Kempei Tai Headquarters.

The green circle is the location of the Headquarters Hospital .

This is followed by the left and right side pages of
a document with dates of death and location.

There are 14 names in this list and
numbers 7 - 12 died within the KMP Hospital.

Next to Cpl Forystek's name is a blue asterisk.

In the right side of this page his
cause of death is stated as third degree burns
