The Missing Air Crew Report ( MACR ) was required to be filed
within 48 hours of a B-29's failure to return to base.

Parts of MACR 12500 are in these two pages.

Below are two versions of the first and second pages.

The first is the original from Fold3 and is difficult to read.

The second is from Sgt Janecek's IDPF.
This is a copy dated 10 April 1946.

Each crew member is listed with their position at
the left side column and their status is in the right side column.

Initially, all of them would be listed as Missing in Action ( MIA ).
This would change as updates became available.

All were eventually identified with
their status changed to Killed in Action.
( Five were identified as a Group )

Sgt Janecek was captured and taken to Tokyo.
An 'illness' resulted in him being transferred to a
Military Hospital where he died.

He was buried in the Koishikawa Military Cemetery.
Upon recovery, his status was changed to DOW ( Died of Wounds ).

Sgt Evans was the only crew member to survive the war.

Paragraph 12 is at the bottom of the first page and has
the name(s) of any person,
in this case crew members from other B-29s,
who have knowledge of the aircraft.

The are four names in this list and their reports are in Page 2.

At the bottom of this page is a list with the names of Next of Kin.
