Cpl Stanley Forystek's journey home began with
a letter from the Quartermaster General to his mother.

Included in this letter was an explanation of the enclosed pamphlets.

Mrs Forystek filled out the form Request for Disposition of Remains and
chose to have her son returned to the United States
with burial in Fort Snelling National Cemetery.

This document was signed 25 October 1948.

Page three is blank as this is the part of the form used when
the Next of Kin relinquishes disposition authority.
